- White cottony patches: Fuzzy, cotton-like growths on the body, fins, or mouth are almost always a fungal infection (Saprolegnia) or Columnaris (a bacterial infection that mimics fungus). These usually appear on fish that are already weakened by injury or poor conditions.
- Floating upside down or sideways: This points to Swim Bladder Disorder. In fancy goldfish, it is often caused by overfeeding or a diet too high in dried foods. In other species, it can signal a more serious internal infection or physical injury.
Water Quality vs. Biological Disease: How to Tell the Difference
A simple rule of thumb: if multiple fish are sick at the same time, suspect water quality first. A disease like Ich or a bacterial infection typically spreads from one fish outward over days. A Note on Medicating Your Tank
Medication should always be a last resort, not a first response. Troubleshooting Guide
1 I treated for Ich but the white spots are not going away after a week.
Possible cause: The medication is likely being neutralised by activated carbon left in the filter, or the water temperature is too low to accelerate the parasite's life cycle out of its medication-resistant stage.
Remove all activated carbon and chemical filter media before medicating — carbon absorbs medication within hours and renders it useless. Gradually raise the water temperature to 28–30°C to speed up the Ich life cycle so the vulnerable free-swimming stage emerges faster. Continue treatment for a full 7 days after the last visible spot disappears, not just until the fish looks clear.
2 I did a water change and my fish got worse, not better.
Possible cause: The new tap water may have a significantly different temperature, pH, or chloramine level from the tank water, causing chemical or temperature shock. Some municipal water supplies also have high chloramine concentrations that a standard dechlorinator does not fully neutralise.
Always match the temperature of new water to within 1°C of the tank before adding it. Use a dechlorinator that explicitly neutralises chloramine (not just chlorine) — products like Seachem Prime are effective at detoxifying both. Never change more than 50% of the water volume at once in a crisis. If the fish worsened after a large change, perform a smaller 15–20% top-up to gradually stabilise chemistry.
3 My fish has been treated but keeps getting sick with the same disease repeatedly.
Possible cause: The underlying cause of the immune suppression has not been identified or fixed. Recurring disease almost always indicates a chronic environmental stressor — most commonly persistently elevated nitrate, an overstocked tank, incompatible tankmates causing chronic stress, or a heater that is causing temperature fluctuations overnight.
Test nitrate and aim to keep it consistently below 20 ppm through regular weekly water changes. Check your heater's accuracy with a reliable thermometer — cheap heaters frequently cycle 2–4°C above and below the set point overnight, which is enough to suppress immunity. Audit the tank's stocking level and social dynamics for bullying. A fish that keeps getting sick in clean water is almost always a fish that is chronically stressed by its environment.
4 My fish is showing symptoms but my water test results look fine.
Possible cause: Strip tests are notoriously unreliable and can show falsely safe readings even when ammonia or nitrite is present. Alternatively, the stressor may be environmental rather than chemical — temperature fluctuation, bullying, incorrect pH for the species, or a subclinical pathogen load that doesn't affect test parameters.
Retest using a liquid drop test kit (API Master Test Kit is the most widely recommended). Check the expiry dates on your reagents — old test solutions give inaccurate readings. Also test your tap water source directly to rule out a contamination issue. If chemistry is genuinely clean, observe the tank for several hours to look for aggression between fish, and verify that the temperature is stable over a 24-hour period using a min-max thermometer.
5 I added medication but now my other fish look stressed and are also at the surface.
Possible cause: Most medications — particularly formalin-based and some antibiotic treatments — reduce dissolved oxygen in the water as a side effect. This can push healthy fish into oxygen stress even if they were not the original patient.
Immediately increase surface agitation by adding an air stone or angling the filter outlet to break the surface more aggressively. Perform a 30% water change with dechlorinated water to dilute the medication concentration and re-oxygenate the tank. If you are not medicating in a quarantine tank, strongly consider transferring the sick fish and stopping medication in the display tank entirely.
6 My fish's fins look ragged but I cannot tell if it is fin rot or nipping from tankmates.
Possible cause: Both fin rot and fin nipping produce torn, uneven fin edges, which makes them visually difficult to distinguish without close observation.
Watch the tank for at least 30 minutes during feeding, when fish are most active and social hierarchies are most visible. Fin nipping leaves clean, sharp tears and typically affects only one fish in a specific area. Fin rot produces a soft, fraying, or dissolving edge that slowly enlarges over days, often with a white or reddish border. If the damage is staying the same size and the fish is otherwise active and eating, it is most likely nipping damage that will regrow. If the affected area is getting larger each day, treat for fin rot with a mild antibacterial.
7 I set up a new quarantine tank for a sick fish but I have no cycled filter media to seed it.
Possible cause: A brand-new filter has no beneficial bacteria and will immediately begin accumulating ammonia from the sick fish's waste, potentially causing ammonia poisoning on top of the existing illness.
Use a small amount of substrate or a decoration from your established display tank to seed the quarantine filter with beneficial bacteria. Alternatively, dose the quarantine tank with a bottled bacterial starter culture (Seachem Stability or similar). If neither option is available, perform small daily water changes (20–30%) in the quarantine tank to manually keep ammonia below 0.25 ppm while the filter establishes. Adding a small amount of Seachem Prime every 24–48 hours will also temporarily detoxify ammonia and nitrite without removing them from the test kit readings.

- Ammonia (NH3/NH4+)
- A toxic nitrogenous waste compound produced directly from fish waste, uneaten food, and decomposing organic matter. Even at 0.25 ppm, ammonia causes gill damage and immune suppression. Safe level is 0 ppm at all times. The most common cause of sudden fish illness in both new and established tanks.
- Nitrite (NO2-)
- The second toxic compound in the nitrogen cycle, produced when beneficial bacteria begin processing ammonia but have not yet fully established. Nitrite binds to haemoglobin in the fish's blood, blocking oxygen transport — a condition sometimes called 'brown blood disease'. Safe level is 0 ppm.
- Nitrate (NO3-)
- The final, relatively less toxic product of the nitrogen cycle, removed through regular water changes. At chronically elevated levels (above 40–80 ppm depending on species), nitrate suppresses immune function and causes long-term health decline. Safe level for most tropical fish is below 20 ppm; below 10 ppm for sensitive species.
- Nitrogen Cycle
- The biological process by which beneficial bacteria (primarily Nitrosomonas and Nitrospira species) in the filter and substrate convert toxic ammonia into nitrite, and then nitrite into the less harmful nitrate. A tank that has not completed this cycle (typically 4–8 weeks) cannot safely house fish without intensive water change management.
- Ich (Ichthyophthirius multifiliis)
- The most common parasitic disease in freshwater aquariums, caused by the ciliate protozoan Ichthyophthirius multifiliis. Presents as small white spots resembling grains of salt across the body and fins. Highly treatable when caught early using a combination of heat (28–30°C) and medication. The parasite is only vulnerable to treatment in its free-swimming juvenile stage, not while attached to the fish.
- Velvet (Oodinium / Piscinoodinium)
- A parasitic disease caused by a dinoflagellate organism that presents as a fine gold or rust-coloured dusty coating on the fish's skin. More aggressive than Ich and harder to see without a torch. Requires copper-based treatment or acriflavine. Particularly dangerous for bettas and gouramis.
- Dropsy
- A clinical symptom rather than a disease itself, characterised by severe bloating and outward-protruding scales (pineconing). Caused by kidney failure and internal fluid accumulation, typically as a result of a systemic bacterial infection. Prognosis is poor once full pineconing is visible. Requires immediate isolation and antibiotic treatment.
- Columnaris (Flavobacterium columnare)
- A bacterial infection that closely resembles a fungal infection in appearance, presenting as white or greyish cottony patches, particularly on the dorsal region and mouth. Spreads rapidly in warmer water and does not respond to antifungal medications. Requires antibacterial treatment.
- Fin Rot
- A progressive bacterial infection (most commonly Aeromonas or Pseudomonas species) that causes the fin tissue to fray, recede, and dissolve from the edges inward. Distinguished from physical fin damage by its spreading nature and characteristic ragged, often discoloured edge. Caused by poor water quality and stress.
- Quarantine Tank (QT)
- A separate, dedicated aquarium used to isolate new fish before introducing them to the display tank (minimum 4 weeks), or to treat sick fish away from the main tank. Prevents medication from damaging the main tank's biological filter, invertebrates, and live plants. A basic QT requires only a small cycled sponge filter, a heater, and a lid.
- Biological Filter
- The colony of beneficial bacteria (Nitrosomonas and Nitrospira) that live in the filter media, substrate, and tank surfaces, responsible for processing ammonia and nitrite through the nitrogen cycle. Most aquarium medications damage or kill this bacteria colony, which is why medicating in a quarantine tank is always preferred.
- Flashing
- A behaviour where a fish darts suddenly and intentionally scrapes or rubs its body against hard surfaces such as rocks, substrate, or glass. Caused by physical irritation of the skin or gills, most commonly from external parasites such as Ich, velvet, or gill flukes. Not to be confused with general erratic swimming caused by water quality issues.
- Swim Bladder Disorder
- A condition where the internal gas-filled organ responsible for buoyancy control malfunctions, causing the fish to float upside down, sideways, or sink to the substrate involuntarily. In fancy goldfish, commonly caused by overfeeding or a diet of dried foods. In other species, may signal internal bacterial infection.
- Osmotic Shock
- A rapid and potentially fatal disruption of a fish's internal water and salt balance caused by a sudden change in the salinity, pH, or mineral content of the surrounding water. Commonly triggered by adding aquarium salt to a tank containing scaleless fish, or by performing a large water change with water of a significantly different chemistry.
- New Tank Syndrome
- The period of elevated ammonia and nitrite toxicity that occurs in a newly set-up aquarium before the nitrogen cycle is biologically established. Typically lasts 4–8 weeks. Managed through frequent partial water changes (30–50% daily or every other day) to keep toxic compounds below lethal thresholds while beneficial bacteria colonise the filter.
- Dechlorinator
- A water conditioner added to tap water before it is used in an aquarium, to neutralise chlorine and chloramine added by municipal water treatment facilities. Chlorine and chloramine are lethal to fish and beneficial bacteria. Products containing sodium thiosulfate neutralise chlorine; products like Seachem Prime are required to also neutralise the more stable chloramine compound.
